Might be worth checking the flag in the document feeder in case its sticking down.
Open the ADF cover as you would to remove some jammed media, you should see the separation pad in the middle, just to the front of it should be the flag.
If it's stuck you can tell by tapping the base of the document tray just near it, if it pops up then it's sticking.
This might be causing the document loaded message, if the flag keeps sticking you need an engineer attend and swap the ADF unit as individual parts are not repaced.
For the other issue, check if you are on the latest firmware and if not upgrade to check if it's a fix in the firmware as the first step.
